Earlier this year, we mailed a preventive care checklist to our plan members. Your Peoples Health patients may bring the checklist to their appointments with you, as we encouraged them to share it with their doctor and use it to guide conversations about services they may need.
The checklist is intended to support proactive planning between your patients and you. It does not need to be returned to Peoples Health. Instead, it serves as a reference to help your patients understand recommended preventive services and engage in shared decision‑making around those for which they may be due.
The checklist includes tests specific to patients with diabetes, including an A1c test (to monitor long‑term glycemic control), an eye exam (to detect diabetic retinopathy and help prevent irreversible vision loss) and statin therapy (to reduce cardiovascular risk). Additionally, an eGFR and a uACR are listed, as these are vital annually for monitoring kidney health.
