Provider Portal Tools to Streamline Care Management

Save time, close gaps and improve quality performance with Member Viewer reports and other vital aids

As a reminder, many tools to enhance your administrative workflows and assist in managing the health needs of your Peoples Health patients are available through the UnitedHealthcare Provider Portal. Log in to your account using your One Healthcare ID to access these key resources:

Use Peoples Health Member Viewer to see your full panel, identify open quality gaps and prioritize outreach.

Choose the Additional Tools menu, then navigate to the Additional Tools section. Once in Member Viewer, select from the Criteria drop-down menu.

  • List All – view your full panel to:
    • Create scheduling priorities (high assessment needs or active management)
    • See last annual wellness visit date and schedule accordingly
    • Identify critical medication refill needs
  • Provider Portal Quality Measure – review open gaps
  • Daily Census – view inpatient admission and discharge reports to coordinate post-discharge follow-up care, including medication reconciliations within seven days of discharge
  • Recent ER Admits with FMC – view acute and observation discharges to focus follow-up on patients at higher readmission risk

Many other reports are available from the menu. These insights help support proactive outreach, improve quality performance and ensure timely care.

Verification of one or more qualifying conditions (diabetes, chronic heart failure, cardiovascular disorders) is required for your Peoples Health patients enrolled in a chronic condition special needs plan (C-SNP) to remain in the plan.

Use the TrackIt tool to attest to qualifying conditions.

  • Select it from the top menu and choose SSBCI (Special Supplemental Benefits).
  • Review your list of patients pending verification. To see all:
    • Select Date Range Search
    • In the pop-up window, choose Last 90 days
    • Select the Attest needed filter and click Apply
  • Then click Attest next to each patient.
  • Based on clinical information in your records, indicate the patient’s qualifying chronic condition or note if none apply.

TrackIt is updated regularly, including to list patients enrolled in a Peoples Health dual-eligible special needs plan, which requires a qualifying condition for access to the food and utilities benefit.

Alternate submission option: If unable to attest through the portal, submit verification by fax using the appropriate form.
