Key areas to improve quality, reduce risks and enhance health outcomes

For 2026, we’re continuing our initiatives focusing on the following care aspects to facilitate optimal health outcomes for your Peoples Health patients:

  • Completing common lab work to monitor health and reduce risk of certain conditions
  • Reducing hospital readmissions through post-discharge medication reconciliation during a follow-up visit or phone call within seven days of a patient’s discharge
  • Completing a Medicare annual wellness visit once per year per patient

Provider Tools

Get a report of your Peoples Health patients with open quality gaps. 
Log in to the UnitedHealthcare Provider Portal with your One Healthcare ID and access Peoples Health Member Viewer. Choose the Additional Tools menu, then look in the Additional Tools section. Once in Member Viewer, select Provider Portal Quality Measure from the Criteria drop-down menu. To view your full panel, choose List All.
