We are conducting a variety of targeted outreaches to our plan members throughout the year to reinforce the importance of key actions, including managing diabetes, scheduling recommended preventive care, adhering to prescribed medications, and understanding medication safety for those taking multiple anticholinergic medications or both opioids and benzodiazepines. These efforts are through mailed communications designed to engage and educate.
As part of this strategy, we are again distributing a personalized health profile to help members better understand and plan their care. We first developed and sent this resource last year and received positive feedback. The health profile presents potential care gaps in a simple chart format and is intended strictly as a resource for tracking preventive care needs.

The health profile is not a form and neither members nor providers need to return it to Peoples Health. The profile is meant as a tool to support productive conversations during provider visits and encourage members to stay on track with recommended care. Tell your Peoples Health patients to keep their profile for their records.
The profile and other communications we send encourage members to speak with their providers about these health topics, including concerns around medications, such as side effects and correct usage. Your patients may reference the materials or ask questions during visits. We encourage you to reinforce these messages and support members in completing recommended screenings and tests.
